We're looking for a motivated, detail-oriented Clinical Coordinator to join our San Francisco office and help us continue delivering exceptional care to the patients and families who depend on us.ABOUT THE ROLEThis is a fast-paced, high-impact position at the heart of our clinical operations. As a Clinical Coordinator, you'll serve as the connective tissue between our field clinicians, patients, families, and physicians — keeping care plans on track, documentation accurate, and communication seamless. If you thrive in a dynamic environment, love keeping things organized, and are passionate about home health, this role is for you.WHAT YOU'LL DO• Coordinate patient care and provide essential administrative support to field clinicians• Track, clarify, and process medical orders, ensuring complete and accurate documentation• Act as the primary liaison between patients, families, and physicians regarding care plans and updates• Input orders and upload documentation into the electronic medical records system• Assist with the onboarding and orientation of new clinical staff• Order and monitor medical supplies to maintain inventory levels and ensure OSHA compliance• Conduct chart audits to ensure accuracy, completeness, and regulatory compliance• Support overall office operations and take on additional responsibilities as neededWHAT WE'RE LOOKING FOR• Current RN license in California (required)• 1+ year of nursing experience; home health experience strongly preferred• Associate degree or higher preferred• BLS certification is a plus• Exceptional verbal and written communication skills with the ability to manage high call volumes professionally• Strong organizational and multitasking skills — you can juggle priorities without dropping the ball• Comfortable with Apple products and proficient in medical scheduling and documentation software• A service-first mindset with genuine warmth toward patients, families, and colleaguesSCHEDULE & LOCATION• Monday – Friday, Day Shift• On-site at our San Francisco office (remote work not available for this role)WHAT WE OFFER• Competitive hourly compensation starting at $50.00/hour, depending on experience• Medical, dental, and vision insurance• 401(k) with employer matching• Life insurance• A collaborative, forward-thinking team culture where your work truly mattersREADY TO MAKE AN IMPACT?If you're a licensed RN who thrives in a coordinator role and wants to be part of a team that genuinely puts patients first — we'd love to hear from you.
