• Established Bay Area home health agency (20+ years)Your Role• Perform occupational therapy evaluations and reassessments
• Develop and manage individualized plans of care
• Deliver skilled therapy focused on ADLs, safety, fall prevention, and independence
• Address home safety and adaptive equipment needs
• Educate patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Complete documentation in KinnserRequirements• Active California Occupational Therapist license
• Valid California driver’s license
• Strong clinical judgment and time-management skills
• Home health experience preferred but not required
