We are currently seeking a compassionate, organized, and team-oriented Medical Social Worker (MSW) to join us on a per-diem basis for the South Bay area.At Health Link, we foster a collaborative work environment. If you are passionate about patient care and thrive in a supportive setting, Health Link is the place for you.Compensation & Benefits•Competitive Pay: $40.00 – $60.00 per hour•Unmatched Field Support: Our experienced clinical team is always ready to assist—whether you need help navigating a complex situation or just need someone to call a patient if you’re running late.•Flexible Schedule: Take control of your work-life balance.•Paid Sick Time: Because your health matters, too.•Mileage Reimbursement: Travel expenses for your field visits are covered.Key Responsibilities•Perform initial evaluations during the admission process to assess the patient's psychosocial status.•Identify socioeconomic and emotional factors that may impact the patient's treatment plan.•Collaborate with the interdisciplinary team to develop and implement comprehensive patient care plans.•Consult directly with attending physicians regarding any necessary changes to treatment.•Maintain and submit accurate, timely clinical documentation (evaluations, care plans, progress notes).•Provide monthly supervision to the social work assistant.Requirements•Current Licensed Clinical Social Worker (LCSW) credential in California.•Valid California driver’s license and reliable transportation.•Current First Aid and CPR certification.Coverage Area:** South Bay and surrounding communities.Create a Job AlertInterested in building your career at Health Link?
