Location: San Francisco OfficeHealth Link Home Health and Hospice, a respected leader in home health care serving the greater San Francisco Bay Area, is celebrating nearly 20 years of providing exceptional care across multiple medical specialties. We are seeking a dedicated and self-motivated Clinical Coordinator to join our dynamic Hospice team. As we continue to provide high-quality care to our patients, we need a driven individual who shares our passion for improving healthcare outcomes and enhancing the patient experience.This immediate opening in our San Francisco office offers a collaborative, fast-paced environment where you'll work closely with clinical and office staff to deliver top-notch care.Key Responsibilities:• Coordinate Patient Care: Support field clinicians by ensuring seamless communication and coordination between patients, families, and the healthcare team.• Order Management: Request, track, clarify, and fax patient orders, as well as upload them into the electronic medical record (EMR) system.• Supply Management: Monitor and order medical supplies, ensuring proper stock and maintaining OSHA-compliant organization in the supply room.• Patient & Family Communication: Act as a liaison to communicate with patients and families about their care plans and needs.• Documentation: Assist with order input, float visits, and obtaining necessary documentation to ensure compliance and quality patient care.• Team Support: Aid in orienting new personnel and assist with audits or other duties as needed to maintain high clinical standards.Why Join Us:• Collaborative Team: Work alongside passionate and supportive professionals dedicated to improving patient care in a forward-thinking company culture.• Competitive Compensation: Enjoy a competitive salary with comprehensive benefits, including medical, dental, vision, 401(k) matching, life insurance, and more.• Room for Growth: Be part of a team that values your contributions and invests in your career development as we approach our 20-year anniversary.Required Qualifications:• Current California LVN License (mandatory)• Experience in Home Health or Hospice settings (preferred)• Strong verbal, written, and interpersonal communication skills• Familiarity with Apple products and comfort with technology• Excellent organizational skills, time management, and attention to detail• Ability to handle a high call volume in a calm, pleasant manner• Knowledge of medical terminology• Ability to work independently and as part of a teamSchedule & Compensation:• Schedule: Full-time, Monday to Friday, Day Shift• Pay Range: $35 - $40 per hour, depending on experience• Benefits: Health, dental, and vision insurance, life insurance, 401(k) with matching, and more!Preferred Qualifications:• Associate’s Degree or higher (preferred)• BLS Certification (preferred)• Experience: At least 1 year in nursing or related clinical field (preferred)Work Location:This is an on-site role in our San Francisco office.
