We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Join Ennoble Care today!Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Bowie, Maryland area. We support internal advancement and long-term career growth based on performance, organizational needs, and role availability.KEY RESPONSIBILITIES• Coordinate care for long-term care residents using Ennoble Care’s EMR• Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions• Collaborate closely with Ennoble Care providers, within the long-term care setting• Serve as a key liaison between providers, facility staff, residents, and families• Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner• Accurately document patient interactions and care coordination activities• Follow established workflows, documentation standards, and escalation protocols• Contribute positively to team collaboration and strong facility partnershipsWHAT 'EMBEDDED' MEANS AT ENNOBLE CAREThis is not a remote or call-center role.As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting.REQUIREMENTS• Legally authorized to work in the United States of America• Active, unrestricted clinical certification or licensure, such as:• Patient Care Technician (PCT)• Certified Nursing Assistant (CNA)• Medical Assistant (MA) / Certified Medical Assistant (CMA)• Licensed Practical Nurse (LPN)• Registered Nurse (RN)• Or comparable credential• Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting• Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person• Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting• Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset• Strong working knowledge of healthcare terminology and care coordination principles• Proficiency with Microsoft 365, including Outlook, Word, and Excel• Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an on-site role, based in the Bowie, Maryland area• No weekends• No on-call• Daytime hours• Seven (7) company-paid holidays annually• Transportation criteria:• Valid, unencumbered driver’s license• Consistent and reliable transportation to fulfill the duties of this position• Current auto insurance, with the ability to provide proof• Please note, travel between facilities may be required• Ability to pass a background check and drug screenFull-time benefits eligibility with a variety of options, including a Wellness Pet Plan for your furry family members!We appreciate your interest in joining Ennoble Care, where To Care is an Honor ❤️Full-time employees qualify for the following benefits:• Medical, Dental, Vision and supplementary benefits such as Life Insurance, Short Term and Long Term Disability, Flexible Spending Accounts for Medical and Dependent Care, Accident, Critical Illness, and Hospital Indemnity.• Paid Time Off• Paid Office HolidaysAll employees qualify for these benefits:• Paid Sick Time• 401(k) with up to 3% company match• Referral Program• Payactiv: pay-on-demand.
