to make a meaningful difference, and we are committed to bringing a culture of caring to every encounter.This is a remote position eligible to those who reside in Alabama, Mississippi, Texas, Louisiana, Oklahoma, Kansas, Missouri, Arizona or FloridaWhere You’ll Make an Impact• Promotes the agency philosophy and mission by presenting a positive image to patients and families.• Demonstrates professional qualities as evidenced by being dependable, punctual, and maintaining a positive attitude, showing enthusiasm, and striving for excellence.• Understands and uses various forms of communication which may include phone system, software, reports, tools, and metrics.• Provides effective communication to patients and families, team members, and other healthcare professionals.• Uses motivational interviewing and active listening skills to understand patient needs and effectively communicate to identify/resolve issues.• Takes detailed notes and enters them into the patient’s electronic medical record.• Follows up with patients as required to ensure their needs have been met.• Maintains confidential medical records for each patient in accordance with HIPAA guidelines.• Stays up to date on company policies and procedures.• Performs all other duties as assignedWhat You Bring• High School Graduate (College Preferred).• If clinical, current state license as a Licensed Vocational Nurse or Licensed Practical Nurse.• One year of general office experience is preferred.• Reliable transportation.Why This Opportunity• Direct influence on patient outcomes and reduction of avoidable hospitalizations• High-visibility role supporting key value-based partnerships and initiatives• Collaborative environment with strong clinical and operational alignment• A growing organization where you can contribute and buildJoin a team where your work truly matters.Apply today and help us transform lives through genuine caring.
